A few days after the events of Resident Evil 7, Resident Evil Village takes place on February 9, 2021. The protagonist, Ethan Winters, returns. Ethan's wife Mia and 6-month-old daughter Rosemary were kidnapped by Chris Redfield and his henchmen and taken to a secret European town, where they were held captive for a few days before being released. To save Rosemary, Ethan must explore the village. Mutants with werewolf-like appearances known as Lycans have taken over the hamlet, which is now ruled by four separate mutant lords, each of which has a fortress inside the community from which to command their own army. Her three mutant female maids, Bela, Cassandra, and Daniela, live in Castle Dimitrescu with Lady Alcina, an abnormally tall vampiric noblewoman. Donna Beneviento controls from her house, House Beneviento, via her puppet Angie, who induces hallucinations. An uncanny "merman" known as Salvatore Moreau works out of a nearby reservoir, and is referred to as such by the locals. Karl Heisenberg, a magnetic field manipulator, commands a troop of modern-day Soldat simulacra. Mother, the town's ultimate leader and "presence revered by the locals," commands the respect of every dwelling in the community.

Chris Redfield has transferred Ethan and Mia to Europe with their newborn daughter Rosemary three years after the events in Dulvey. During a raid on the residence, Chris and his Hound Wolf crew kill Mia and kidnap Ethan and Rosemary. Lycans, werewolf-like animals, attack a neighboring community. Ethan wakes up next to the vehicle they were travelling in. To rescue the surviving villagers, Ethan is abducted by Mother and her lords, including Alcina Dimitrescu, Donna Beneviento, Salvatore Moreau, and Karl Heisenberg, and brought before them. In order to locate Rosemary, Ethan evades a trap set by Heisenberg and enters Dimitrescu's castle with the help of a local trader known as the Duke. The head of Rosemary is in a flask that Ethan uses to take out Dimitrescu and her daughters. Rosemary's body parts were put in four distinct flasks for a specific ceremony, and Ethan must collect the other flasks held by the other lords in order to bring her back to life.

In the midst of slaying Beneviento and Moreau, Ethan comes upon Hound Wolf. Ethan passes a Heisenberg test for the fourth flask and is invited to the lord's factory, where Heisenberg proposes that they work together to destroy the slayers. As soon as he discovers that Heisenberg plans to use Rosemary as a weapon, Ethan decides to flee and does so. On the day that Mia was slain, Ethan meets and confronts Chris, discovering that Chris had disguised himself as a woman named Miranda to kill Mia. During a conversation with Miranda, Chris admits that she has the ability of imitation and was trying to kidnap Rosemary by crashing the vehicle she was on. Chris devastates Heisenberg's facility, while Ethan defeats him with an improvised tank. After revealing her intentions to claim Rosemary as her own, Miranda confronts Ethan and murders him.

A Bioterrorism Security Assessment Alliance assault team distracts Miranda as Chris takes Hound Wolf to Rosemary. Entering a cavern, Chris finds the source of the mold, a Megamycete and a "Black God" (the Japanese game's name). Oswell E. Spencer's mentor, Oswell E. Spencer, was mentored by the Megamycete and utilized her expertise to produce the t-Virus, which he planted a bomb on the Megamycete and discovered in her laboratory. Miranda tried to resurrect her daughter Eva, who had died of the Spanish flu, by using the fungus; the four lords, Lycans, and Eveline were all unsuccessful trials. As a result of Rosemary's particular talents acquired from Ethan and Mia, Miranda was able to choose an appropriate host. When Mia discloses her husband's talents, Chris is able to free both Ethan and Chris from their respective prisons.

It is revealed that Ethan was murdered by Jack Baker in Dulvey, but was reborn thanks to Eveline's mold, which gave him the ability to regenerate. To resuscitate Eva, the Duke invites Ethan to a ritual location where Rosemary was revived instead. After Ethan murders Miranda, the Megamycete erupts from the earth and attacks him. In order to explode the bomb on the Megamycete and save Mia and Rosemary, Ethan, whose body is decaying due to the limits of his regenerative talents, gives his life. After discovering that BSAA troops dispatched to the hamlet were organic bioweapons, Chris orders his unit to march for the BSAA's European headquarters.
